The Creative Movement, Dance & Physical Education Enrichment Program is a 45-minute small-group physical education class designed for students in 3rd through 5th Grade. This engaging and inclusive program combines dance, movement, fitness, rhythm, coordination, and creative expression in a fun, supportive learning environment. The class welcomes typical learners, gifted students, neurodiverse learners, and twice-exceptional students.

Led by our highly-trained instructor, Ms. Michelle Alonzo, students participate in age-appropriate movement activities that promote physical fitness, body awareness, coordination, balance, flexibility, strength, and motor skill development. Through guided movement experiences, dance routines, creative expression activities, rhythm exercises, and cooperative games, students build confidence while developing healthy movement habits and a positive attitude toward physical activity.

Each class incorporates elements of creative movement, introductory dance techniques, fitness activities, stretching, balance exercises, locomotor skills, rhythm exploration, and choreographed movement sequences. Students are encouraged to explore movement in a safe and encouraging environment while improving listening skills, following directions, self-regulation, teamwork, and social interaction.

Hands-on activities may include movement storytelling, rhythm and music activities, obstacle courses, fitness challenges, creative movement games, dance routines, and prop-based movement experiences designed to keep students actively engaged. Lessons emphasize participation, effort, creativity, and personal growth rather than competition.

This educational enrichment program provides students with meaningful opportunities to develop physical literacy, movement confidence, and social-emotional skills while fostering a lifelong appreciation for health, fitness, music, and creative expression.

Authorization for Emergency Medical Treatment • Photo Release • Injury Liability Waiver

I understand that in the case of illness or injury to my child or children listed
above, Ave Maria Arts and Innovation Circle LLC will try to notify me or the
person I have listed above as an emergency contact. In case of medical emergency
concerning my child, at a time when I or my listed emergency contact cannot be
notified, I grant full power to Ave Maria Arts and Innovation Circle LLC to 1)
arrange for the transportation of my child, whether by ambulance or otherwise, to
a proper facility where emergency medical treatment would normally be
administered, including but not limited to, an emergency room of a hospital, a
doctor's office, or a medical clinic; and 2) sign releases as may be required in order
to obtain any medical or surgical treatment as is required in the judgment of
medical authorities at the facility.

Photo Release

For good and valuable consideration, the receipt of which is hereby acknowledged,
I hereby grant Ave Maria Arts and Innovation Circle LLC (AMAIC) permission to
use the likeness of my family in a photograph in any and all of its publications,
including but not limited to all of Ave Maria Arts and Innovation Circle LLC
printed and digital publications.
I understand and agree that any photograph using my family's likeness will
become the property of the AMAIC and will not be returned.
I acknowledge that since my participation in Ave Maria Arts and Innovation Circle
LLC is voluntary, I will receive no financial compensation.
I hereby irrevocably authorize AMAIC to edit, alter, copy, exhibit, publish, or
distribute this photo for purposes of publicizing Ave Maria Arts and Innovation
LLC or for any other related, lawful purpose.
In addition, I waive the right to inspect or approve the finished product, including
a written or electronic copy, wherein my likeness appears.
Additionally, I waive any right to royalties or other compensation arising or related
to the use of the photograph.
I hereby hold harmless and release and forever discharge Ave Maria Arts and
Innovation Circle LLC from all claims, demands, and causes of action which I, my
heirs, representatives, executors, administrators, or any other persons acting on
my behalf or on behalf of my estate have or may have by reason of this
authorization.

Injury Liability Waiver

Participation in any physical activity creates the possibility or risk of serious
injury, including permanent paralysis and even death. In consideration of being
permitted to participate in the Ave Maria Arts and Innovation Circle LLC as well
as any outside play for recreational benefits to my child(ren), I hereby waive and
release any and all claims for damages I may have against Ave Maria Arts and
Innovation Circle LLC and all the owners, agents, teachers, tutors, contractors,
and employees, free and harmless from any loss, damage, liability injury,
negligence, cost of expenses that may be suffered by my child(ren) while
participating in the activities of the program or class. I understand that
participants in the Ave Maria Arts and Innovation Circle LLC are not covered by
any insurance held by Ave Maria Arts and Innovation Circle LLC. I agree to be
completely financially responsible for all medical expenses arising of my child(ren)
participation or observation in any program or class offered and agree to carry
adequate medical insurance. I agree that I will abide by all rules, regulations, and
conditions as prescribed by Ave Maria Arts and Innovation Circle LLC and the
participating venue.
